Hills Digital Photography Workshops
Please read the terms below carefully, complete the form at
the bottom of the page and click the submit button. Upon
submitting your form you will be taken to the checkout page for
payment. Thank you!
Participants
are required to register in advance for each workshop, registrations
will NOT be accepted at the door. In addition, each participant must
have signed the release form included with the registration materials.
If this release form is not signed, your registration will be
not
be accepted. We
also reserve the right to cancel a given workshop if not enough
students are enrolled. The decision to cancel a
workshop due to enrollment issues will be made 7 days
before the workshop. If a workshop must be cancelled by us,
full
refunds will be provided to each person enrolled in that workshop.
Our
Refund Policy: Your tuition fee may be refunded to
you provided that written notice of cancellation is received at the
registration address below at least fourteen (14) days before the
workshop is scheduled to begin. The full amount less a $25.00 handling
fee will be refunded to you within thirty (30) days. No refunds
are given for cancellation requests received less than fourteen (14)
days before the scheduled workshop.
Liability
Waiver and Assumption of Risk: Flint Hills Digital
Photography Workshops are
designed to provide each participant with a safe, educational and
enjoyable
experience.
Personal
Risk:
By participating in this outdoor workshop you acknowledge that there
are
certain risks and dangers associated with and directly related to
participation
in such events and/or outdoor activities. These dangers include, but
are not
limited to, natural disasters, forces of nature, outdoor terrain,
extreme
changes in temperature, wild or otherwise uncontrolled animals, other
participants or people, vehicular travel and modes of transportation
including
walking, hiking, and climbing.
Equipment
and Vehicle Risk:
By participating in this outdoor workshop you acknowledge that there
are risks
associated with exposing your photographic equipment and vehicle to the
elements.
These
dangers include, but are not limited to, natural disasters, forces of
nature,
extreme changes in temperature, submergence in water, sand, dust and
wind.
Assumption
of Risk:
The participant assumes all responsibility for any accident or
accidents, personal loss, personal injury or damage to property
incurred by you
while travelling to the workshop, during participation in the workshop
or while
travelling from the workshop. Scott Bean (Scott Bean Photography), Wayne
Rhodus (Rhodus Photography), & the Columbian Theatre/Swogger Gallery their heirs, assigns, or any other
beneficiaries
will assume no liability for injury, illness, or loss of personal
property or
expenses thereof associated with workshop participation. By signing
this
document and remitting your payment you declare that you are in good
physical
condition, are in good health and are capable of participating in an
outdoor
photography workshop. Further you acknowledge that you are aware of the
inherent risks identified above and hereby waive all claims for
liability
against Scott Bean (Scott Bean Photography), Wayne Rhodus
(Rhodus
Photography), & the Columbian Theatre/Swogger Gallery their heirs, assigns, or any other beneficiaries as the
result of
your participation in this workshop.
I
have read this document in its
entirety. I understand the risks of participation and fully agree to
the terms
and conditions of the Liability Waiver and Assumption of Risk. I
further
acknowledge that I am in good physical condition and that I am
physically
capable of participating in an outdoor photography workshop, and
consent to all
terms and conditions as stated above. By making my payment I
am certifying that I have read the above terms and agree
with the above statements.
